These cysts develop due to <strong data-start=\"466\" data-end=\"490\">degenerative changes<\/strong> in the spine and may compress nearby nerves, causing <strong data-start=\"544\" data-end=\"563\">lower back pain<\/strong> and <strong data-start=\"568\" data-end=\"601\">radiating leg pain (sciatica)<\/strong>.<br data-start=\"602\" data-end=\"605\" \/>While some facet cysts cause no symptoms, others can lead to <strong data-start=\"666\" data-end=\"703\">spinal canal narrowing (stenosis)<\/strong> and significant discomfort.<\/p>\n<h3 data-start=\"733\" data-end=\"787\">How Common It Is and Who Gets It? (Epidemiology)<\/h3>\n<p data-start=\"788\" data-end=\"1074\">Synovial facet cysts are uncommon but typically occur in <strong data-start=\"845\" data-end=\"877\">older adults (over 50 years)<\/strong> due to arthritis and wear-and-tear changes in the spine. They are most frequently found at the <strong data-start=\"973\" data-end=\"988\">L4\u2013L5 level<\/strong>, where motion and degeneration are greatest. They are surrounded by a capsule that produces <strong data-start=\"1275\" data-end=\"1293\">synovial fluid<\/strong>\u2014a lubricant that helps the joints move easily.<br data-start=\"1340\" data-end=\"1343\" \/>With aging and degeneration:<\/p>\n<ul data-start=\"1374\" data-end=\"1829\">\n<li data-start=\"1374\" data-end=\"1452\">\n<p data-start=\"1376\" data-end=\"1452\"><strong data-start=\"1376\" data-end=\"1412\">Intervertebral discs lose height<\/strong>, putting more stress on facet joints.<\/p>\n<\/li>\n<li data-start=\"1453\" data-end=\"1533\">\n<p data-start=\"1455\" data-end=\"1533\"><strong data-start=\"1455\" data-end=\"1474\">Facet arthritis<\/strong> develops, leading to inflammation and joint instability.<\/p>\n<\/li>\n<li data-start=\"1534\" data-end=\"1829\">\n<p data-start=\"1536\" data-end=\"1829\">The capsule may produce excess synovial fluid, which can form a <strong data-start=\"1600\" data-end=\"1618\">pocket or cyst<\/strong>.<br data-start=\"1619\" data-end=\"1622\" \/>This cyst may press on nearby <strong data-start=\"1652\" data-end=\"1667\">nerve roots<\/strong> or the <strong data-start=\"1675\" data-end=\"1691\">spinal canal<\/strong>, causing pain and neurological symptoms.<br data-start=\"1732\" data-end=\"1735\" \/>Occasionally, <strong data-start=\"1749\" data-end=\"1797\">bleeding into the cyst (facet cyst hematoma)<\/strong> causes rapid symptom worsening.<\/p>\n<\/li>\n<\/ul>\n<h3 data-start=\"1831\" data-end=\"1889\">How the Body Part Normally Works? (Relevant Anatomy)<\/h3>\n<p data-start=\"1890\" data-end=\"2241\">The <strong data-start=\"1894\" data-end=\"1910\">facet joints<\/strong> are located behind each vertebra and guide motion in the spine. Each joint has smooth cartilage and is enclosed by a capsule filled with synovial fluid for lubrication.<br data-start=\"2079\" data-end=\"2082\" \/>When degeneration occurs, the capsule may weaken, bulge, and form a cyst. Some patients may need repeat injections.<\/p>\n<\/li>\n<\/ul>\n<h4 data-start=\"4600\" data-end=\"4620\">Surgical Care<\/h4>\n<p data-start=\"4621\" data-end=\"4759\">Surgery is considered when symptoms persist despite conservative treatment or neurological deficits develop.<br data-start=\"4729\" data-end=\"4732\" \/>Surgical options include:<\/p>\n<ul data-start=\"4760\" data-end=\"5187\">\n<li data-start=\"4760\" data-end=\"4936\">\n<p data-start=\"4762\" data-end=\"4936\"><strong data-start=\"4762\" data-end=\"4796\">Laminectomy and decompression:<\/strong> Removes part of the bone and facet joint to relieve nerve pressure and remove the cyst. Usually done for one-sided (unilateral) symptoms.<\/p>\n<\/li>\n<li data-start=\"4937\" data-end=\"5187\">\n<p data-start=\"4939\" data-end=\"5081\"><strong data-start=\"4939\" data-end=\"4967\">Facetectomy with fusion:<\/strong> Involves removing part or all of the facet joint and stabilizing the spine with screws and rods. Physical therapy is initiated to improve muscle strength and flexibility. <a href=\"https:\/\/2025divi.cortho.org\/general\/heat-cold-and-compression-therapy\/\">Heat and cold therapy<\/a> may help relieve the pain and stiffness in some patients.<\/p><p>Epidural injections may also help in the temporary control of pain. In patients where the symptoms clearly correlate with the synovial facet cyst, a CT guided cyst rupture and cyst steroid injection may be done. Repeat injections may be required in some patients.<\/p><p>Operative management is indicated in patients with persistent symptoms despite conservative management. Surgical management may be done in the form of <a href=\"https:\/\/2025divi.cortho.org\/spine\/lumbar-laminectomy-surgery\/\">lumbar laminectomy and decompression<\/a>. The surgery consists of creating an opening in the lamina and the facet joint to take out the cyst. Laminectomy and decompression surgery are generally done in patients with unilateral symptoms.<\/p><p><a href=\"https:\/\/2025divi.cortho.org\/spine\/lumbar-fusion\/\">Facetectomy with fusion surgery<\/a> is the treatment of choice when indicated.
